Short answer. A trial-ending reminder says when the trial ends, what the first payment will be and how to cancel. Send it before the first full-price payment, inside the window you promised at checkout. Copy the example and fill in the brackets.
The template
Subject: Reminder: your free trial ends on [date]
Hello [first name],
Your free trial of [plan name] ends on [date]. After that we will charge [amount] every [interval] to your [payment method].
If you want to carry on, you do not need to do anything.
To manage or cancel before then: [cancel link]

Short trials
If the trial is shorter than your reminder window, the reminder can fall before sign-up. The calculator flags this. Send it at sign-up or shorten the window.
